Abstract: Strengthening capacity building at the grassroots level is a key part in the coordination between disease prevention and control agencies and hospitals relating to high blood pressure and diabetes treatment. Based on the analysis of the survey and interview results made in medical and health institutions at various levels, it is found that there are still problems at grassroots-level medical and health institutions such as the lack of motivation for prevention and control due to the separation of medical and prevention funding, excessive emphasis laid on medical care rather than prevention and control in assessment work, the shortage of professional personnel, and insufficient information integration and sharing. It is suggested to carry out health-centered integration of prevention and treatment and whole-process management of chronic diseases at the grassroots level through reform of mechanisms such as incentive promotion, assessment improvement, proper arrangements for medical personnel and technology advancement. First, we need to establish a financing and payment mechanism for the integration of medical treatment and prevention with a health-oriented incentive to encourage the retention of fund balances. Second, we need to expedite the forming of management and assessment indicators for integrating prevention and treatment of high blood pressure and diabetes. Third, we need to take multiple measures to form a multidisciplinary chronic disease management team. Fourth, we need to integrate various information systems at the grassroots level and improve the assessment of referral cases of medical treatment in medical consortiums. Fifth, we need to accelerate the application of innovative technologies to improve the decision-making capacity of primary-level medical institutions.
